Die sogenannte Serotonin- oder auch Monoamin-Hypothese, die auf der Annahme basiert, dass ein Mangel an Serotonin und möglicherweise auch von anderen Monoaminen wie Noradrenalin und Dopamin ursächlich für eine Depression ist, ist mittlerweile umstritten. Das liegt vor allem daran, dass diese Hypothese nicht erklären kann, warum nur ein Teil der Patienten auf Antidepressiva anspricht oder warum das Ansprechen mit zeitlicher Verzögerung erfolgt. Die Wirkung von Antidepressiva beruhe möglicherweise auf einem starken Placeboeffekt oder auf einem allgemein dämpfenden Effekt auf das emotionale Empfinden, aber vermutlich nicht auf einer Erhöhung des Serotonins. Man geht mittlerweile davon aus, dass es sich bei der Depression nicht um ein einheitliches Krankheitsbild handelt, sondern um eine Sammlung verschiedener Erkrankungen mit ähnlichen Symptomen.
Kommen Sie jetzt bitte nicht auf die Idee, Ihr Antidepressivum einfach wegzulassen. Erstens wirkt es, zweitens kommen Sie unweigerlich wegen der Absetzphänomene ins Schleudern. Wenn absetzen, dann nur langsam und nur in Zusammenarbeit mit Ihren BehandlerInnen.

Depression is a complex, heterogeneous disorder with biological, psychological, and sociocultural determinants and risk factors. Historically, psychiatrists have never explained clinical depression solely in terms of reduced serotonin or any specific neurotransmitter. As with SSRIs, many drugs in clinical medicine work through unknown or multiple mechanisms, and this does not affect their safety, efficacy, or approval for medical use. There is ample evidence from placebo-controlled studies that serotonergic antidepressants are safe and effective in the treatment of acute major depressive episodes. If serotonergic agents are not helpful, antidepressants from other classes may be considered.

The paper, and the response to it, reveal the still-existing chasm between what mental health professionals say they know and what the general public understands about what causes depression, and how it’s treated. There are two separate questions being raised. One is: Do depressed people have different levels of serotonin? The review suggests the answer is no. The second question is: Do SSRIs work to help some people with symptoms of depression? “The second question isn’t addressed in the paper,” Foulkes said. “The danger is, it’s now being used to answer that question.”
